IMPORTANT DOCUMENTS
PassHello wonderful 2019 Staff! Below you will find the hiring documents needed for this summer. Please read THIS before doing anything else!
Here are the documents you need to fill out and upload to the Wufoo site listed below for employment at Big Lake for 2019:
- Employment Info Verification
- 2019 W4 (has to be done in person, cannot upload document)
- 2019 Form OR-W-4 (only need to fill out if you want more taxes withheld this year)
- I-9 paper version expires 8-31-2019 (has to be done in person with original documentation, cannot upload)
- Direct deposit form (only fill out if you want your payroll directly deposited to your bank account)
- Employee Data Collection (do not have to fill out, but write, “Decline to fill out” at top of page and upload)
- OR Sick Leave notice-posting
- 2019 Staff packing list (do not need to fill out anything on this or upload)
- Parents of minor staff members (only needed for staff who are under the age of 18)
